Is this just breathing hard into a piece of plastic?

No — and the difference is the entire point. Breathing hard with no resistance is cardio for muscles that are already easy to satisfy. The calibrated ring forces your diaphragm and intercostals to generate real pressure against a measured restriction, which is what drives adaptation.

How fast will I notice anything?
